What is Teeth Clenching?

Bruxism covers both grinding and clenching, but they are not quite the same thing and they do not always occur together.
Grinding involves a lateral movement of the jaw, sliding the teeth back and forth against each other. It tends to happen during sleep and is more likely to cause visible wear on the biting surfaces of the teeth.

Clenching is different. It is a static action, holding the teeth together under sustained, forceful pressure with no movement. It can happen during sleep, but it is also extremely common during waking hours, often triggered by stress, concentration or emotional tension.

Because clenching involves no movement, it generates a different kind of damage.

The sustained pressure loads the teeth vertically rather than wearing them laterally, which can cause cracking, fracturing, increased sensitivity and significant stress on the roots and supporting bone.

It also drives chronic overactivity in the masseter muscle, which over time becomes enlarged and increasingly powerful, making the clenching harder to control and more damaging with every episode.

Understanding which pattern you have matters for treatment. Patients who primarily clench rather than grind often find that a night guard provides limited relief, because a guard is designed to protect against grinding friction rather than absorb the sustained load of a sustained clench.

Botox addresses the root cause by reducing the force the masseter can generate, which is why it works where other approaches fall short.

How Teeth Clenching is Resolved With Botox.

The masseter is the large muscle on either side of the jaw responsible for chewing and, in bruxism patients, for the forceful clenching that causes so much damage and discomfort.

When it is overactive, it becomes overdeveloped, generating levels of pressure the teeth and jaw joints were never designed to sustain.

What Happens If Clenching is Left Untreated?

The masseter is one of the strongest muscles in the human body relative to its size. In people who clench regularly, it is working far harder than it should be, often for hours each day and night without the person realising. Over time this has predictable consequences.

The muscle itself becomes overdeveloped through hypertrophy, the same process that causes any muscle to enlarge with repeated heavy use. This can produce a noticeably squarer or more pronounced jawline. More significantly, the sustained force is transmitted directly to the teeth, the periodontal ligament, the jawbone and the temporomandibular joint (TMJ).

The TMJ is not designed to absorb this level of continuous loading, and in chronic cases it leads to TMJ disorder, characterised by joint pain, clicking, limited range of motion and referred pain into the ear and temples.

Catching and treating clenching early prevents this progression. The longer the masseter is allowed to operate at this level, the more established the pattern becomes and the more damage accumulates.
