The Bruxism Quiz and other online assessmen
We offer a free online quiz and other self-assessment tools on the Website (together, the “Quiz”). The Quiz is delivered through a third-party form platform (YouForm) and the information you submit is passed to our customer relationship management system (GoHighLevel) so that we can contact you with your results and, if you request it, with follow-up information about our services.
You acknowledge and agree that:
- the Quiz is a general self-assessment tool and is not a medical diagnosis. It does not, and cannot, confirm whether you have bruxism or any other condition.
- the Quiz results are indicative only and should not be relied upon as a substitute for a clinical assessment by a qualified practitioner.
- by submitting the Quiz you consent to your responses and contact details being transmitted to, and processed by, YouForm and GoHighLevel on our behalf for the purposes of providing your results and any follow-up communications you have consented to.
- if you do not wish to share your information with these providers, please do not submit the Quiz — you are welcome to contact us directly by phone or email instead.
- we are not responsible for the availability, accuracy, or performance of the underlying third-party platforms, although we take reasonable steps to choose reputable providers and to protect the information you submit.

Bookings, consultations, and treatments
Any bookings, consultations, or treatments arranged through the Website (including via the Quiz, contact forms, callback requests, or booking widgets) are subject to:
- availability;
- our acceptance of your booking;
- a satisfactory clinical consultation and suitability assessment;
- any clinic policies in force at the time of booking, including our cancellation, deposit, and rescheduling policies, which will be communicated to you at the time of booking.
Prices shown on the Website are indicative and may change. The price applicable to your treatment will be confirmed at the time of booking. Where we offer payment plans (for example, through third-party finance providers such as Klarna or PLIM), those arrangements are between you and the provider and are subject to their own terms and eligibility criteria.
Botulinum toxin (Botox) is a prescription-only medicine in the United Kingdom. It can only be prescribed following a face-to-face consultation with a qualified prescriber, and treatment will only proceed where clinically appropriate.
